Your insurance requires specific documentation before it will pay any benefit for diabetic shoes and inserts. The packet below will assist you in obtaining the necessary documentation from your physician(s) in order to satisfy your insurance provider’s requirements.
PLEASE READ THE PACKET CAREFULLY. Provide pages to your doctor(s) as it instructs. Once we have received all necessary documentation in our office, we will contact you to schedule your appointment.
